Insurance and Referral Requirements

Most insurance companies cover self-referral to physical therapy.

Most insurance companies provide the same coverage whether you self-refer or obtain a referral from your primary care provider. Workers’ compensation claims, motor vehicle accidents, and Medicare are exceptions. Before your first appointment, our office staff will let you know if your insurance has any special referral requirements.

Major Plans

Performance Physical Therapy accepts Medicare, most major insurance plans, and private-pay clients.

In-Network Coverage

The practice is in-network with major insurers including Aetna, Anthem, Cigna, ConnectiCare, Oxford Health Plans, and United Health Care.
